WFC Project REACH is hiring Program Support
Specialists. This is a per diem position.
Summary:
Per diem staff provide flexible, on-call support to ensure the safety, supervision, and well-being of young adults ages 18–22 residing in a Transitional Living Program. This position fills coverage needs during the days, afternoons, holidays, and staff absences. Per diem staff support daily program operations, model independent living skills, and assist residents in maintaining a safe, stable, and supportive living environment. The program is looking to hire 2-3 per diems.
Case Managers will maintain a strengths-based, trauma-informed approach and work collaboratively with program staff and community partners to ensure holistic support for each participant. The program is looking to hire 2-3 per diems that can:
- Maintain individualized service plans in collaboration with program participants.
- Conduct regular one-on-one meetings to monitor progress toward goals and provide ongoing support.
- Connect residents with community resources related to housing, education, employment, mental health, and healthcare.
- Support participants in developing life skills, such as budgeting, job readiness, and conflict resolution.
- Contribute to a safe, inclusive, and respectful environment within the housing program.
- Collaborate with residential and outreach staff to ensure coordinated, consistent support across all program components.
- Participate in case conferences, team meetings, and training sessions.
- Maintain accurate and timely documentation in the Homeless Management Information System (HMIS) and internal databases.
Experience
- Experience working with at-risk youth, young adults, or individuals experiencing homelessness.
- Understanding of trauma-informed care, harm reduction, and positive youth development.
- Strong communication, organization, and relationship-building skills.
- Ability to work independently and collaboratively within a team.
- Flexibility to work occasional evenings or weekends as needed.
- Knowledge of local housing and social service systems.
- Experience using HMIS or other case management databases
Salary: $20.00 per hour
Qualifications:
- High school diploma or GED (Required)
- Case management: 1 year (Preferred)
- Working with individuals who are unhoused: 1 year (Preferred)
